PEAK is in-network with a variety of insurance companies. To verify if PEAK is in-network with your insurance plan for services, please review the list of insurance companies below.

If PEAK is in-network (or certified) with your insurance company:  Please contact your insurance company to confirm what care is covered by your plan as well as information on copayments, deductibles, and coinsurance amounts.

If PEAK is not in-network with your insurance company:  This means PEAK is considered out-of-network with your insurance plan which can lead to greater out-of-pocket expenses for your care. Please contact your insurance company for more information about what your financial responsibility may be if you choose to receive care at PEAK. Even if we are out-of-network, we are happy to file a claim on your behalf.
